Report duplicate records to OCLC on a CONSER Duplicate Record Report form.
The form provides columns to list the retained records and the corresponding duplicate records. In the first column, write OCLC control number of the retained record. In the numbered columns list the OCLC control numbers of the corresponding duplicates. Supply your OCLC institution symbol and the date submitted in the appropriate areas at the top of the form.
No accompanying documentation is necessary. Also, it is not necessary to fill the page before submitting it. Participants should submit duplicate reports as frequently as possible.
